Capitol Maze held in Sacramento, CA, October 15, 16, and 17.

NOTE: If you choose not to use this online option, you may download the brochure and mail your check or credit card information directly to CSAOHN, address given on the brochure.

Special Offer:
3 for 2 Discount Three "members" who register for the full conference package (excluding preconference
& special event) are eligible for discount. When two members pay the full conference rate, the
third person receives a complimentary registration. All attendees must register at the same time via this on-line
form in order to be eligible for this discount. (This means 3
complete forms turned in at the same time)

Cancellations
Cancellations must be made in writing and postmarked by 9/29/09. No telephone cancellations will be accepted.
An administrative fee of $25 is charged for ALL cancellations. Refunds are dispersed in the same manner as
payment and will be sent within 30 days following the CSAOHN Annual Conference.
